below is a digital copy of the health and safety checklist used on my shoot day - there are 3 ticks for 3 models.

Before

Model patch test form completed and
  reactions documented, contradications checked  |

Put all personal belongings away and
  make sure there are no trip hazards |

Wash hands, loose clothing and
  jewellery  
removed and hair tied back |

Makeup and hair brushes disinfected,

Electrical equipment has been PAT
  tested and cleaned and visual checks undertaken |

Put chair at correct height and put
  gown on model |

Keep a bowl out for rubbish and
  continually put rubbish into it |

Put lids back on products after using
  them |

Make sure model is comfortable
  throughout and keep prosthetics away from nostrils |

Have extractor fan on when using IPA

Help remove the prosthetics on the
  model carefully using the correct products |
